By completing the online joining process, you are registering to participate in activities held by Kate Campbell Fitness, and you are accepting this waiver and all the terms and conditions (insofar as they are applicable). You should only join if you have read them and voluntary accept them.

The participant acknowledges that there can be certain risks of injury to them or third parties associated with their participation in personal training sessions, program, courses, bootcamps, gym sessions machines and class in house or online.

You should always consult with your doctor before beginning any type of exercise or program of physical activity. This form is an important legal document. It explains the risks you are assuming by beginning an exercise program. It is critical you read and understand it completely

The participant should seek professional advice if they have known medical conditions or injury prior to the session, any nutritional information is a guide only if you have any medical conditions or an eating disorder are underweight pregnancy or breastfeeding, then you will need to discuss with your health care professional before you participate.

All clients must fill out the relevant screening forms online.  

For most people, physical activity should not pose any problems or hazards. Screening Forms have been designed to identify the small number of people for whom physical activity might be inappropriate or those who should have medical advice concerning the type of activity most suitable for them.

When filling out the screening form read them carefully and answer honestly. If you answer yes to any of the questions more details are required. You may be asked to seek medical advice or approval before taking part. If your health changes you might be asked to refrain from further exercise until you have spoken with your healthcare professional. 

It is important that you listen to your own body, and work at your own pace, it is vital that you stay hydrated and take a rest whenever you need it. You must make the instructor aware of any medical conditions or injuries. If at anytime you have questions, feel unsafe or unwell you must inform the instructor immediately and discontinue further participation in the activity. 

You are fully aware you are free to withdraw from the activity at anytime and are participating of your own free will. You agree to follow instructions given by kate campbell Fitness trainers and agree to abide by the rules for participating in various activities. 

The instructions and advice presented are in no way intended to be a substitute for medical counselling or advice. Not all exercises are suitable for everyone and this or any other exercise program may result in injury. 

Any user of the exercise programs assumes the risk of injury resulting from performing the exercises shown. The creators, producers, participants, instructors and distributors of this program disclaim any liability or loss of connection with the exercises and advice contained herein. 

I, hereby state that I wish to participate in the activity, and I realise that my participation in these activities involves potential risk of injury, including but not limited to bodily injury, heart attack, stroke, or even death. I also recognise there are other risks associated with exercise and that it is not possible to list everyone. I know and understand the risks of injury which could occur by reason of my participation in any session or programme. I hereby state that I am in good health and able to take part in the activities. I take full responsibility for my own health and wellbeing and agree that any instructions received will be used at my own risk. 

You agree that in the event you are injured or suffer damage you will bring no claim legal or otherwise against Kate Campbell Fitness, Kate Campbell, its employees, self employed freelance instructors, personal trainers, agents and representatives. 

My acceptance also acknowledges my consent to be videotaped, audio recorded and/or photographed during the class, and grants the instructor the sole right to own, use and modify the images and/or recordings for any and all purposes worldwide, including but not limited to advertisements, marketing, and all commercial purposes and in all forms of media, without any compensation (including royalties) in perpetuity. I waive any right to inspect or approve any images or recordings and waive any rights of ownership to the same.
